The Accepted Support for Personal Autonomy (ACAP) is an innovative project to Delay/prevent the institutionalization of older people, dependents, or disabilities.
This program facilitates comprehensive care by connecting and strengthening existing community support with qualified professionals and technological systems, so that people can continue living in their homes with life and quality for as long as possible, generating evidence of the benefits and sustainability of the care model. .
Target Audience
- People who retain their autonomy but may sometimes need or require assistance.
- People with disabilities.
- Older people with addiction or support needs for activities in daily life.
- People with health problems to benefit from greater control and prevent complications.
- People with fragility, alone or with insufficient family support.
Assistance Services to be Offered
Mobility Support
Assistance indoors and outdoors, sharing the person’s location in case of emergency.
Home Support
Prevention and detection of risk situations by analyzing the person’s activity at home.
Health Support
Monitoring the person’s health to prevent complications, informing their family.
Accompaniment
We provide accompaniment, support, and care for activities of daily living.
Program Cost and Duration
The project Has no costs for users (It is funded by the recovery, transformation, and resilience plan).
It starts now and Will run until June 30, 2025.
